Heart disease and stroke are the major causes of death in Michigan…and lifestyle factors contribute to more than half of all deaths.
Factors Contributing to Death
50% - Human Biological Factors, Environmental Factors, Inadequacies in Healthcare
50% - Smoking, High Blood Pressure, High Blood Cholesterol, Alcohol/Drug use, Physical Inactivity, Poor Nutrition, Overweight
A Statewide Cardiovascular Disease Prevention Program that allows employers with less than 500 employees the benefit of comprehensive health benefits for all their workers. The program is funded out of tobacco taxes levied on each sale. The proceeds are funneled into the state where the monies are put back into the communities with high risk populations.
Consideration for the grants are based on the following criteria:
- Have employee groups at higher risk for premature death and disability from Cardiovascular Disease
- Are less likely to start programs without financial and technical assistance
- Have under 500 employees
- Are just starting health promotion activities
Grant: A maximum amount per participant and per worksite is available. Depending on the county for which the business resides between $3,000 and $5,000 per worksite is available

- Cardiovascular Disease Risk Screening (CVDRS)
- Fitness Evaluation/Physical Activity Prescription
- Health Risk Appraisal (HRA)
- On- Site Follow- Up (One Year) for CVDRS and HRA
